An exciting opportunity has become available for a 12 month fixed term/secondment post within the team for a band 5 community nurse. The ALD South Tees Community Team provides services across both Middlesbrough and Redcar and Cleveland based at Flatts Lane Centre and is made up of Nursing, Psychiatry, Psychology and AHP's (including Physiotherapy, Occupational Therapy, Speech and Language Therapy) staff. The team works collectively and collaboratively with others to ensure a high standard of care is provided and are always willing to go above and beyond what is expected when required. The team works in partnership with key partners to ensure people with a diagnosed learning disability and their families and carers are supported by delivering high quality clinical care. The service runs 24 hours a day seven days a week with the expectation to cover 8-8 Monday through to Sunday and be part of an On-Call service from 5pm to 9am. Nursing applicants should already hold NMC registration. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process. Main duties of the job

An exciting opportunity to join a highly experienced multi-disciplinary team within the Teesside locality working and supporting people with Learning Disabilities to achieve high quality of life and optimum health. The successful candidate will hold and manage a case load, working collaboratively with service users, their families, carers, and partner agencies. The post holder will require excellent communication skills and be calm, confident, and resilient in their approach, and will need to be able to form therapeutic relationships. The post holder should be flexible, adaptable, and able to travel independently during the course of their duties. Qualifications

Current professional registration with Nursing & Midwifery Council RNMH or RNLD (RNSC - CAMHS only) - RGN (If appropriate) To be recognised practice assessor/mentor or work towards in an agreed timescale Trained Clinical Supervisor (within 6 month probationary period) Evidence of Continuing Professional Development Experience

Working with people with learning disabilities/mental ill health Working in a person-centred therapeutic organisation Working collaboratively with service users and their families/carers Working in a multi-disciplinary team Working in partnership with other agencies Knowledge

Evidence-based knowledge of best practice in caring for patients in the designated field Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act) Understanding of Safeguarding and its application in practice Clinical Risk Assessment and Management and its application in practice Clinical Governance and its application in practice Confident working knowledge of clinical medicines management, including administration of injections Disclosure and Barring Service Check
