Job overview We welcome applications to join us at the Community Mental Health Assessment Team. The Post Holder will be responsible for the triage/assessment of routine referrals to the assessment team, signposting to other agencies when appropriate and to the wider CMHT. They will hold a short term caseload and be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will provide high standards of skilled mental health care to individuals and their families. The role will include active participation in the service planning and development of the nursing and multi-disciplinary teams. The post holder will provide clinical supervision and support to junior members of the team and actively promote the values and policies of the service and the Trust. The Community Mental Health Nurse will be an active member of the multi-disciplinary team and will work in close cooperation with secondary and primary care based professionals, other community agencies such as Housing, Probation, Police, Children Schools & Families and other Voluntary and Third Sector Agencies. Community Mental Health services are fully integrated teams that are operated jointly with Bradford Metropolitan District Council and all staff within the teams will be expected to deliver health and social care outcomes for service users. Main duties of the job To manage a caseload of clients in the community To triage and assess routine referrals into the CMHT To input data in the clinical information system: e.g. including risk assessments, care plans, CPA, crisis and contingency plans To work as a part of duty/day co-ordinator service within the CMH AT To supervise and mentor student nurses To demonstrate effective networking skills and ensure a holistic approach to meet service user needs To work to ensure the safety and wellbeing of service users and their families To work in partnership with service users and carers to achieve goals and to support recovery and reintegration within communities To provide appropriate feedback and liaison with MDT on all aspects of service user care To ensure that agreed standards of nursing care within the team are achieved To actively participate in the flexible formats of team supervision and support To initiate, develop and assist in the trial of new ideas for the improvement of services to service users in conjunction with the multidisciplinary team To ensure that nursing policies are implemented and maintained To comply with legal and professional requirements To have ability to identify, facilitate and support developments and initiatives within the service delivery of nursing and multi-disciplinary team To work within a fully integrated and jointly managed Community Mental Health Team alongside Local Authority social care staff and managers. Person specification Qualification Current full UK, NMC Registration Experience At least two years post-registration nursing experience. Experience of working in a community mental health setting. Excellent standard of professional communication skills.
