The North Community Mental Health Service is undergoing an exciting transformation with additional resource allocation to provide ongoing intensive support to the population of Sheffield. This includes working towards a joint approach with Primary Care and the Voluntary Sector, reviewing the CMHT Service Criteria, and working in discreet small teams in local communities, leading to a high-quality care standard.

We are looking to recruit a Band 6 Nurse to the North CMHT. A number of our service users require a man as their named worker due to the nature of the service provided, including trauma and risk history. We intend to appoint a man to this post as an occupational requirement because we do not have any men in nursing roles in this service. The successful candidate will be supported with their professional and leadership development through dedicated supervision with highly experienced and motivated team managers, where learning opportunities will be supported and encouraged. From the multi-disciplinary team, we offer regular CPD sessions and candidates are encouraged to bring their own interests to the team to share and develop their specific interests in mental health. Please note this is a qualified Nursing Vacancy.

Main duties of the job You will need to possess excellent communication skills and be able to work independently and as part of the wider team in order to assess, plan, and evaluate care. You should have a positive approach to meeting the needs of our service users and be motivated, empowered, and enthusiastic to both learn and share. You will also be expected to work as part of the duty system which operates within the general working hours of 09:00 to 17:00 Monday to Friday on a rotational basis. This 'mini team' is supported by other clinical professionals and management.

Job responsibilities The post holder will work within a multi-disciplinary service and core responsibilities will include: Acting as a Named Worker Comprehensive assessment of new referrals Carrying a varied caseload defined by need Contributing to a duty system on a rota basis Delivering supervision to more junior members of staff Contributing to Multidisciplinary service development

Clinical Triage and determine urgency of new referrals to the service Carry out comprehensive assessments of an individual's health and social care needs in a variety of settings and agree on the most appropriate course of action. Assess, evaluate, and manage clinical risk Formulate a plan of care based on identified need Deliver evidence-based interventions as appropriate Review the effectiveness of care plans at regular intervals Work within safeguarding processes, including where required to act as safeguarding investigator/manager Collaborate with service users, carers, and partner agencies Maintain computer-held care records in an accurate and timely fashion Undertake all work in a culturally sensitive manner Act in accordance with Care Management System, Acute, and Schedule Care Pathways as appropriate Develop effective working relationships with Primary Care colleagues

Person Specification Knowledge and skills Essential Working knowledge and understanding of Care Programme Approach & Care Management Knowledge and understanding of appropriate therapeutic approaches and engagement strategies Knowledge and understanding of PDR and supervision processes as a supervisee Demonstrable risk assessment and risk management skills with evidence of positive risk-taking

Desirable Additional skills/training in psychosocial interventions for people with SMI, CBTp, and family interventions Relevant Health or Social Care professional qualification - Nursing or Social Work

Training and qualifications Essential Relevant Health or Social Care professional qualification - Nursing or Social Work Current registration with NMC, HCPC Evidence of continued professional development

Desirable Mentorship qualification Supervision training

Experience Essential Experience of multi-disciplinary team working Extensive experience in mental health in both inpatient and community settings Experience of managing/supervising others Experience of working in a recovery-focused way

Desirable Experience of working with adults in acute/crisis/relapse presentation Experience in a Community Mental Health Team/Assertive Outreach team
