9am to 5pm , however offering flexibility as we offer a hybrid model of working and flexible working arrangements. We require motivated individuals, with good initiative and organisational skills. You will work as part of a multi-disciplinary team providing interventions for working age adults. You will ensure that the service users' recovery goals are identified and central to their formulation and care planning needs. Main duties of the job
With access to appropriate clinical & management supervision the post holder will have or will develop & will be able to demonstrate in practice a range of appropriate skills to include assessment (including risk assessment), formulation, safety planning & clinical interventions. The role provides opportunity to coordinate the care of service users, throughout their engagement with the service & wider services if these are required. To achieve this the post holder, through a range of defined competencies will acquire all of the skills necessary to carry out role. This will include a requirement to practice care co-ordination skills through shadowing opportunities & supervision. Workload will be continually negotiated through supervision & case load management with the appropriate clinical team managers. You will work as a Care Coordinator & be an integral part of the Community Mental Health Team. Undertaking shared roles & responsibilities such as initial assessments, care co-ordination & team duty. In addition you will be required to carry out specific assessments & interventions. You will be expected to develop and deliver complex service user focussed Care Plans & provide excellent packages of care, further supported by a Multi-Disciplinary Team & Formulation process. You will also be expected to employ leadership within your role & will provide Clinical Supervision, Preceptorship & support to junior colleagues. The post holder will through a range of defined competencies acquire all of the skills necessary to progress into the role of care coordinator. This will include a requirement to practice care- coordination skills in the clinical setting. To seek out and engage in management and clinical supervision with senior clinicians and clinical team managers. To establish priorities in workload in partnership with senior team members and ensure effective use of time and resources. To maintain a commitment to person centred care and to promote the use of the recovery model where appropriate. To support and carry out comprehensive holistic mental health assessments, formulation, safety plans, the planning and implementation of appropriate care and outcome measures. Qualifications
Relevant Core Professional Qualification – i.e. Diploma OT/BHSc OT, Dip SW, RN (Mental Health), or other relevant qualification Experience
Experience of working with people with ongoing and mental health needs. Experience of working with people at risk. Can demonstrate experience of formulating care plans for people.
