We are developing new community pathways to support care delivery in people’s homes, with various learning and development opportunities available through the CLCH Academy.
We are looking for a part-time community nurse deputy team lead with a genuine interest in developing skills in this field. Flexibility according to service needs and excellent organisational, time management, and communication skills are essential.
Working hours: 4.30pm to 8.30pm
You will be based at one of four sites within the Herts Valley area: Watford, Hertsmere, St Albans and Harpenden, and Hemel Hempstead.

Main Duties of the Job The Community Nurse Deputy Team Lead (DTL) will provide general and specific care to patients under the guidance of a registered nurse or allied health professional, working in patients' homes and residential settings.
Responsibilities include becoming proficient in clinical practice, promoting independence and individualised client-centred care, supporting the Community Nursing Team to meet key performance indicators, and maintaining service quality. The Community Nurse DTL will work 4.30pm-8.30pm across seven days for continuity of care handover to the Overnight services.
