Community Nurse for Children with Disabilities

To work within a multi-disciplinary team closely with staff from other agencies to promote and maintain high standards of care for children and adolescents with learning disabilities / developmental delay and complex needs. This will involve assessing developmental / associated health needs, planning, implementing and evaluating appropriate programmes of care. Where required more specialist assessment will be undertaken under the indirect clinical supervision of Senior Nurse or Team Leader. Such work will be underpinned by ordinary life principles and evidence based practice. The post holder will be expected to work with children, their families, carers, staff teams and other professionals in a variety of community settings with ongoing support. In addition, the post holder will assist with service development activities within the team.

The post holder will be expected to work with children, their families, carers, staff teams and other professionals in a variety of community settings with ongoing support. In addition, the post holder will assist with service development activities within the team.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
