We have an exciting opportunity for a Senior Occupational Therapist to join our Integrated Community Team based in the Cotswold Locality.

This post will have a focus on Home First & Reablement, facilitating hospital discharge and working with patients to develop and achieve their rehabilitation goals.

The ICT consists of Occupational Therapists, Community Nurses, Physiotherapists, and a Home First/Reablement service. We work closely with a range of teams from within our own organisation and key local system partners including, Adult Social Care, GPs, Residential homes, Acute hospitals, Community hospitals, housing and voluntary organisations.

The ethos of the team is a rehabilitation, person-led approach to recovery and self-management, to ensure we offer care close to home, avoiding unnecessary admissions to bed based services and supporting timely discharge from hospital.

Main duties of the job

Applicants will need to demonstrate skills in clinical triage and experience of delivering urgent responses for rapid assessment of patients' needs, planning and providing timely rehabilitation or compensatory strategies/aids. Our philosophy is that supported self-management is key to personalised and positive outcomes for people.

Registered with HCPC. Diploma or Degree in Occupational Therapy or equivalent.

Broad range of clinical experience across a variety of clinical areas Solid experience as an occupational therapist, and the ability to demonstrate relevant experience in the field of rehabilitation Ability to demonstrate judgement in complex clinical/operational problems, which require investigating, analysing, assessing and resolving Relevant training and experience of assessment of complex Moving and Handling

PROFESSIONAL / MANAGERIAL / SPECIALIST KNOWLEDGE

Essential

Relevant experience in managing risk and evidence of ability to make decisions as part of a team, regarding people in crisis. Demonstrate leadership skills in relation to motivating team and resolving operational and personal problems Ability to explain complex information, in relation to assessed needs, including adaptations, legislation and Care Act 2014. Demonstrate an empathy and interest in clients/and their occupational therapy needs.

Desirable

Awareness and understanding of the implications of legislation surrounding confidentiality - the Data Protection Act etc. Ability to fit, assess for and demonstrate a range of standard and specialist equipment, including assessment and fitting of appropriate moving and handling equipment such as hoists, slings, or seating assessments.
