Home First Team
Locum contract for 3 months initially
5 days over seven days (Monday - Saturday: 9am - 8pm / Sunday: 10am - 6pm)
£25 - £27p/h (Mileage paid at 40p per mile)

Our NHS Client are recruiting a Band 6 Community Occupational Therapist for their Home First Team in Stoke, for a 3 month locum contract initially.
The Team supports adults within their own homes where the primary need is physical disability with long-term health conditions.
Key responsibilities:
Assess complex care/support needs and develop, implement, and evaluate tailored care packages for service users across community and hospital settings.
Prevent unnecessary hospital admissions and contribute to improving population health and wellbeing.
Have supervisory, training and leadership responsibilities where appropriate and will have some responsibility for team performance and service.
Assess service users and formulate, develop, implement and evaluate programmes of treatment/care/support for individuals, families and carers.
Requirements:
Have a degree in Occupational Therapy and be registered with the HCPC.
Have post qualification experience within a social care or community setting.
