An exciting opportunity has arisen to join this dynamic and innovative community team providing specialist interventions to children and young people who have a Learning Disability, their families and carers. Applicants are invited for the post of Community Learning Disability Nurse – Band 5 within the Paediatric Learning Disabilities Service in Bolton. Applicants are required to be a Registered Nurse (Learning Disability), RNLD. We are looking for enthusiastic and confident applicants who show a commitment to the delivery of high-quality services in a variety of community settings, with individuals, carers, and families. They will be able to provide assessment, carry out interventions, and give advice to our service users and other agencies to empower and build resilience. The service works to promote the health and wellbeing of the children and young people that are referred to the service, developing opportunities to meet identified health outcomes in order for the children and young people to be included in all aspects of their school and family life. You will have the opportunity to develop knowledge and skills and work across wider Children's Services as needed. You will be responsible for the assessment and interventions of children on your caseloads alongside Parents, Carers, Education, Local Authority, and other health professionals. You will be able to work in collaboration with health, education, and social care and work to support early help for families. Main duties of the job
Job Summary To work autonomously within a multi-disciplinary framework. To manage a defined caseload of children with Global Development Delay/Learning Disability and additional health needs under supervision. To provide assessment and delivery of high standards of care and treatment in the home and within a variety of community settings. To professionally contribute to the service performance targets, objectives, and clinical governance strategy. It is desirable that candidates can demonstrate or are willing to develop skills and knowledge in Positive Behaviour Support, sleep, continence, diet and nutrition, and puberty and have experience of working with children and families. Communication and Relationship Skills To communicate information and breakdown barriers to understanding for clients, family, and significant others. To utilise appropriate resources to ensure understanding has been achieved. To communicate effectively with the children and parents/carers to explain clinical issues. To use appropriate communication techniques when dealing with children who may present with challenging behaviours. To communicate effectively and work collaboratively with health and other professionals to ensure the delivery of a co-ordinated multi-disciplinary package of care. To communicate assessment and observations with regards to learning disabilities and associated needs to all relevant professionals involved in the care of the child/young person. To provide advice to clients, carers, other professionals, and groups to promote understanding of the Paediatric Learning Disabilities Service. To demonstrate effective inter-professional working practices that respect and utilise the contributions of multi-agency professionals. Knowledge, Training and Expertise To complete assessment of children with learning disabilities and additional needs, which is evidence-based, systematic, accurate, and relevant. To develop, implement, and evaluate appropriate episodes of care, treatment based on best available evidence, within a framework of informed consent. To contribute to the training of students and junior staff. To adhere to the Code of Professional Conduct and work within the Scope of Professional Conduct Guidelines as per Nursing and Midwifery Council. To undertake Continuous Professional Development including clinical supervision activities, in order to enhance knowledge, skills, values, and attitudes needed for safe, effective and evidence-based practice. To participate in the staff appraisal and Personal Development Plan. To keep up-to-date and develop knowledge with other professionals. Professional: To support the co-ordination, planning, implementation of the services within the community, taking into account the changing needs of the service users and the organisation. To be aware of and act in accordance with agreed policies and procedures of the organisation. To support the day-to-day management of the team. To provide advice and support to team members working with Children with Learning Disabilities. To contribute to the development of policies and procedures in relation to the delivery of the service. To participate in peer supervision, using critical appraisal and reflection to ensure that own practice is evidenced-based. Planning and Organisational: To accept clinical responsibilities for a designated caseload, and to organise this effectively and efficiently with regards to clinical priorities and use of time. To plan and deliver group/individual activities as appropriate. To be actively involved in the collection of appropriate data and statistics as required. To be aware of Health and Safety Practice, to work to organisational policies and to promptly report and record incidents/accidents as appropriate. To comply with the organisational and departmental policies and procedures and to contribute to the reviewing process.
