To be responsible for the clinical development of the palliative care support team in the effective delivery of care packages & highly skilled nursing care for patients with complex palliative care needs. To play a key role in ensuring that all relevant parties involved in the delivery of care have adequate training and support and that patients have access to appropriate services. To have day to day management responsibility for the Palliative Care Support Team. To work in collaboration with all appropriate services to facilitate a rapid, seamless discharge to enable PPOD. To work alongside the co-ordinating lead to support service development and transformational changes.
