Community Palliative Nurse (Hospice Rapid Access Service)

St. Luke's Hospice is a charity rooted in the local community and dedicated to providing compassionate care to individuals facing incurable diseases. Our goal is to preserve dignity and provide patient choice during the final stages of life. We are now exploring an opportunity for an experienced and devoted registered nurse to join our Innovative Hospice Rapid Access Service. This provides a rewarding opportunity for someone who is passionate about delivering high-quality palliative care services. Main duties of the job

You will be instrumental in coordinating the safe and timely discharge of Palliative patients from hospital to their preferred place of care. Working collaboratively with our Hospice Collaborative Partners & ICT teams to holistically assess patients in their own homes, or in the inpatient environment, to ensure people are able to achieve their wishes at the end of life. Working in conjunction with a multi-disciplinary team, you will offer support to patients, relatives and healthcare professionals throughout the pathway. Internally the role is known as Rapid Access and Discharge Clinician. Job responsibilities

Developing an effective discharge plan in collaboration with the hospital, GP and community services and that Hospice rapid access funds are applied appropriately. Visiting patients in their place of residence. Providing necessary care, support and advice to patients, family members and other healthcare professionals and ensuring patients are reviewed in a timely manner. Contributing towards ongoing quality improvement of palliative care services by participating in audit and research activities. Working cohesively within a multidisciplinary team to ensure effective communication and optimal patient care. Collaborating with Inpatient Unit regarding use of Rapid Access Beds. Ensuring smooth transition of care provision when ending the Hospice Rapid Access service pathway. Person Specification

Experience

Demonstrated clinical experience. Awareness of palliative care. Strong interpersonal and communication skills. Excellent organisational skills with the ability to manage complex situations. A commitment to the values and mission of St. Luke's Hospice. Qualifications

NMC Registration. Disclosure and Barring Service Check
