An exciting opportunity for a Physiotherapist who has experience of, or is interested in community based working, has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care so we are able to work together in a fully integrated way.

The successful applicant will join a dynamic, friendly team of therapists, community nurses and social care staff. You will be providing Physiotherapy to patients in the community and have the chance to work in an autonomous manner carrying your own caseload of patients with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. The role includes provision of therapy for patients with long term conditions but you will also be providing a responsive, same day service to patients to facilitate hospital discharges and carry out joint health and social care assessments of need in the community. On site at the health centre you have access to a range of treatment facilities and well equipped gym to provide individual and group therapy.

Main duties of the job You will be using a wide range of Physiotherapy skills and will need high level assessment skills. Experience of neurological conditions, musculoskeletal conditions, frailty, falls, dementia and orthopaedics will be an advantage. You will have opportunities to develop your role in accordance with areas of clinical interest, training and education are available to support this. We work closely with our Quality Improvement team and there are many opportunities for you to initiate projects which will lead to service improvement. Your additional duties will include the supervision and mentorship of junior staff and students and you will be involved in ongoing learning and development within the team.

This is a full time post. You will need to be a car owner and driver. We operate a 7 day service within core hours 8am to 6pm.

JOB PURPOSE: To be an active member of a multi-disciplinary, integrated community team.

To manage a defined caseload, using evidence based/person centred principles to assess, plan, implement and evaluate therapy interventions.

To supervise band 3 and 4 staff.

Person Specification Education and qualifications Essential Current HCPC registration Degree in Physiotherapy Up to date manual handling training for the lifting and manoeuvring of clients and equipment during treatment sessions

Experience and knowledge Essential Completed clinical placements as a student Documented evidence of continuing professional development

Desirable Clinical placement in a community setting Post registration experience in a clinical setting Advice and support to students and support staff.

Skills and abilities Essential Demonstrate good IT and standard keyboard skills. Able to work flexibly to meet the needs of the service Highly motivated Good observational and reporting skills Able to plan and provide appropriate levels of clinical intervention Ability to analyse professional and ethical issues. Ability to work within a team setting. Ability to reflect on own performance. Demonstrate empathy and sensitivity Frequent Travel Essential - You will need to travel, so you must either hold a full, current driving licence and have access to personal transport or meet the mobility requirements of the role through other reasonable and suitable means.

Desirable Training and facilitation skills Supervisory skills. Able to support other staff to review and reflect on their practice and develop a PDP Knowledge of mental health legislation including CPA and risk assessment and management. Evidence of working at a relevant level Knowledge of working within the NHS

Personal qualities Essential Commitment to person-centered, non-discriminatory practice. Positive approach to change Ability to work in domestic environments with low levels of hygiene and potential exposure to fleas, lice and body fluids. An understanding of hazards associated with lone working in the community. Manoeuvre limbs of around 5-6 kg Manoeuvre patients using handling aids
