Job summary We are looking to recruit new Community Physiotherapists to join our friendly Therapy team in Erewash. We have hours available to offer full and part-time roles, providing opportunities to work within various workstreams including Rapid Response, Higher Priority, and Routine core. You will see patients in the Community, allowing you to adopt agile working between your home and our two bases: Long Eaton Health Centre and Ilkeston Community, offering great flexibility and independence in managing your caseload. The Core Community Therapy team comprises Physiotherapists and Occupational Therapists, with band 4 and band 3 support, as part of a large Integrated Community Team that includes Community Nursing, Care Co-ordinators, and the Erewash Care Team. We work closely with Rapid Community Therapy, and you will be required to work weekends as part of a shared rota, allowing you to see urgent patients as part of a wider team. You will receive enhanced weekend pay and days off during the week as a result. You will join an established team with a wealth of experience and have access to a Clinical Lead Physiotherapist who will support your development. Main duties of the job You will conduct comprehensive, holistic assessments in the Community setting. Community is viewed as a specialty in its own right, exposing you to a range of conditions including Falls, MSK, Neurology, Respiratory, manual handling, and equipment provision, allowing you to develop your knowledge and skills with a focus on Frailty. You will establish treatment plans and goals following your assessment and manage your own caseload. Collaboration with your Core Community colleagues will be necessary to manage patient flow and maintain capacity within your diary. You will attend meetings, including Community Delivery Team (CDT) meetings with our GP practices, enabling you to develop good working relationships with the relevant Care Co-ordinator for the practice you are linked to. Mandatory training related to your role will be required, and we will identify any gaps in your knowledge and skills to support you in gaining competencies as a Community Physiotherapist.
