We are looking for an excellent Community Practitioner who has patient care at the heart of their practice. If you are a committed Practitioner with the desire to deliver the highest standard of care and treatment, we would be delighted to hear from you. Main duties of the job

Be responsible for the Assessment and Management of service users who have substantial and complex needs. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, and 3rd sector partner agencies. Develop Recovery and Wellbeing focused Care Plans in collaboration with the Service User and Carer. Consider Carer needs and ensure Carer and family involvement in care planning. Deliver evidenced based clinical and psychologically based Interventions. Work as a member of the Multi-disciplinary Team ensuring high quality individualised care. Coordinate Treatment Planning & Interventions, Reviews and Discharge Planning. Provide clinical supervision to staff as delegated by Pathway Clinical Lead and Pathway Manager. Provide compassionate care based on empathy, kindness, respect, and dignity. Incorporate a client centred philosophy into nursing practice. Person specification

Knowledge and Experience

12 to 24 month band 5 qualified nurse experience dependent upon service need. Awareness and understanding of professional accountability, NMC standards and other local and national initiatives. Experience of working with adults with complex mental health needs. Education and qualification

Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist. Practice Assessor Training. Evidence of post-registration training in clinical psychological interventions/skills relevant to the post. Proven track record of post registration clinical experience relevant to the post. Up-to-date knowledge/experience of relevant legislation. Experience of working in a community team. Qualified Non-medical prescriber. Skills and Competencies

Proven ability to carry out and provide comprehensive written assessment of individuals with complex mental health needs. Understanding of effective and appropriate treatment options. Clinical leadership. Role/Team specific requirements

Able to maintain professional development. Experience in working in a multi-disciplinary team. You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
