We have an opportunity for a Band 6 Community Practitioner to join the Psychiatric Liaison Team based at Sunderland Royal Hospital. We offer a 24/7 service into a multi-site acute care hospital.

This is an interesting and varied role, working in fast-paced environments including Accident and Emergency and acute in-patient wards.

You will be responsible for undertaking biopsychosocial assessments, including in-depth risk assessments, delivering brief interventions, and liaising with other health and social care services, educational organisations, and others involved with all age groups.

Liaison services provide the acute Trust staff with expert advice, as well as ongoing support and treatment to patients, as required.

In Liaison Psychiatry, staff undertake comprehensive biopsychosocial mental health assessments alongside robust risk and collaborative safety planning.

When patients remain in the acute hospital for medical treatment, we provide liaison interventions, which may include brief psychological interventions.

You will liaise with professionals involved in the person's care, including family and carers, acute care staff, social care, and other CNTW services.

We have a supportive MDT each day with psychiatrists, psychologists, pharmacy, and our Occupational Therapist, providing feedback on assessments and liaison interventions.

Key Responsibilities:

Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs. Risk Assessment and Risk Management / Safety Planning skills. Demonstrate an understanding of effective & appropriate treatment options including a range of evidence-based clinical approaches as identified in N.I.C.E. Guidance.
