The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible. Delivered in people's own homes within the community, the service makes every effort to prevent unnecessary admission to hospital and facilitates the opportunity for people to return home following a hospital admission or period in residential care. Support students and more junior staff within the team through clinical supervision and mentorship, and may be asked to line manage some staff. Work within a dynamic multi-disciplinary health team, demonstrating sound evidence-based practice and excellent communication and interpersonal skills. Manage complex cases in partnership with other agencies. About us
