To support the delivery of the Healthy Child Programme (HCP) 0-19 (DH, 2010) thereby improving outcomes for children and young people and to the Public Health agenda, meet Public Health targets and reduce health inequalities in partnership with key agencies meeting National agendas.
Promote the protection of children, adhering to the Local Safeguarding Children's Partnership policies and procedures. Working under the supervision of a band 6 Health Visitor/School Nurse within a designated area, alongside partner agencies such as Action for Children, The Children's Society, Family Hubs, Children's Services, and education systems.
Work as part of a team to ensure that Key Performance Indicator (KPI's) targets for the service are met, to support service development, standard setting, clinical audit and benchmarking, delivering a high-quality need-led, safe and effective service to all client groups.
Main duties of the job
Participate in the assessment of health needs and formulation of care plans for children, young people, their families and their communities. This includes the implementation of programmes of care, offering support and advice to parents and families in the home, clinic, school or community setting. In collaboration with the public health nursing team, identify individuals and groups within the community where collaborative working and advice would forward the interests of public health and promote healthy living, particularly seeking to identify disadvantaged groups and address health inequality. Offer advice and support to children, young people and adults in accordance with evidence-based practice. Actively take responsibility for appropriately identifying, accessing, and participating in supervision, according to individual need (e.g., child protection, CAMHS, managerial). Support the delivery of KPI's by working collectively across the area. Under the direction of the Specialist Community Public Health Nurse (SCPHN), complete Early Help assessments, and work with the multi-agency team to support those children, young people and their families in need of additional support and those at risk of significant harm, including participating in Team Around the Family meetings, and supporting Child Protection Meetings (where appropriate). This includes undertaking the role of lead professional or key worker for children and their families with the most complex needs.
