The role:

The role has become available to join a committed and motivated team who support individuals when they are ready to be discharged from hospital. The team works hard to deliver excellent standards in a positive working environment. The Hospital Team works in partnership with individuals to identify what matters to them. This includes undertaking Mental Capacity Assessments, Continuing Health Care Assessments, and leading Best Interest Meetings to ensure people have a safe and timely discharge from hospital. We work alongside a range of professionals within the teams, including colleagues from the health board, therapies, and external providers. It is an exciting time to join adult social care with friendly, experienced, and supportive colleagues who are dedicated to providing a service that puts citizens at the center of our work. An engaging welcome into the team with support and guidance from peers, colleagues, and management when you commence your new role, which will continue throughout your career within Social Work at NPT Council. Meet & Greets with the team are available prior to interview to give you an opportunity to find out more about the hub, services, and what it is like working within the team on a daily basis. There are also opportunities for more experienced social workers, such as training to become an AMHP or Practice Educator, and enrollment on management and leadership training courses. NPT Council invests in their staff, this is an opportunity to join a local authority that will support your development. Ideally you will have:

Applicants will need to be social work qualified and professionally registered as a social worker with Social Care Wales. You will hold knowledge and ideally working practice of the Social Services and Wellbeing (Wales) Act 2014, to include outcome-focused assessment and care planning, knowledge and working practice of Continuing Health Care, the Mental Capacity Act 2005 including Deprivation of Liberty Safeguards.
