Wirral Community Health and Care Trust are seeking a highly motivated and dynamic individual, who is compassionate and has a passion and enthusiasm for palliative care to join our community specialist care team as a Senior Nurse Practitioner. The service is multi-professional, with an Occupational Therapist, Physiotherapists, Dietitian and Social work professionals working directly as part of the team, with nurse availability 7 days per week.

The job is Full time 37.5 hours per week.

Main duties of the job We are looking for a motivated individual with experience in palliative/specialist palliative care who has an understanding of current drivers in the development of palliative/specialist palliative care services and is eager to be part of a dynamic award-winning multi-professional team.

Person Specification Qualifications and professional training Essential Registered General Nurse educated to degree level (or equivalent) Current NMC registration Principles of Pain Management module or equivalent experience Care of the Dying patient/family module or equivalent experience Evidence of recent study/further professional Development

Experience and knowledge Essential Community Experience 2 years' experience in a palliative care setting Knowledge and experience of managing patients with long-term conditions Experience in symptom control and management Ability to manage complex cases

Skills and attributes Essential Team Player Excellent Communication Skills, written and oral Effective interpersonal skills Flexibility and adaptability with the capacity to respond positively to changing situations Ability to exercise initiative Ability to prioritise and manage workload to meet the needs of the service Able to demonstrate commitment to high-quality care and service provision Demonstrates an understanding of National, Regional and local palliative care priorities specific to role

Desirable Computer literate

Computer literate Essential Passion and drive to deliver person-centred care Access to a car for work purposes
