An annual appraisal and personal development plan. DCHS pledges to engage, involve, and support staff to reach their full potential. Supportive Mentorship and Preceptorship. To have involvement in team meetings and the opportunity to make valuable contributions to the team meetings, staff forums, and via Staff Surveys and Pulse Checks. The opportunity to influence and help make changes in the service, to use innovative technology, and to make a difference.

Our purpose is to provide Community Health Services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes, and, increasingly, in people’s own homes and via virtual consultations.

Key Responsibilities

To be part of a highly motivated team, providing a high standard of clinical care. To assess, diagnose, develop, and implement individualised care packages in order to maximize ulcer prevention for patients with a wide variety of clinical needs, including acute or chronic neuropathies, chronic disease, vascular problems, and wound management. The post holder will also be expected to plan and manage their own caseload, which will include providing specialist care to patients with diabetes, rheumatological issues, and biomechanics. As part of the Podiatry Team and Wider Trust, contribute to the development and adhere to the Podiatry Department and Trust policies, strategies, and objectives.
