We have the opportunity for a Band 5 Community Staff Nurse role within the Adult Community Nursing Single Point of Access (ACNT). The team in Bury are looking to recruit a Band 5 with a wealth of clinical skills and knowledge and be flexible and innovative to join their very supportive and extremely knowledgeable team. Support begins at the point of recruitment and throughout your role to gain competence and confidence.

The ACNT team triage all referrals for District Nursing and the Specialist Palliative Care team. Within this role, you will use all your skills and knowledge to communicate with both referrer, patients, and carers in order to ensure a safe and timely response to care. This is a dual role where, on occasions, you will provide a priority response/unscheduled care service to the registered population of Bury. You will visit patients in their own homes including residential homes and assess acutely unwell priority patients.

The post holder should have good communication skills with the ability to prioritise workload responding to patient priority and need.

This is a part-time position (15 hours). The post holder will work weekends and bank holidays, with the flexibility to cover the service until 8pm at weekends. The service runs 7 days a week, including bank holidays and weekends. Shift patterns can be negotiated to support a work-life balance, e.g., long days.

Main duties of the job

The post holder should have good organisational and assessment skills with the ability to prioritise workload responding to patient priority and need, and be flexible to cover the needs of the service. The service does provide an element of twilight cover from 5pm, this will be covered as part of a rota but the post will mainly cover the day element of the service which commences at 8.00 am to 5.00 pm. Shifts will cover weekends and bank holidays.

You will work as part of the clinical team involved in the planning, developing, and evaluation of programmes of care to the practice population and will carry out clinical skills using evidence-based practice.

This vacancy is open to those who have a current NMC PIN to practice as a Registered Nurse in the UK, or those who are currently enrolled in a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

The NCA can no longer accept applications from International Nurses who are yet to complete all competencies needed to receive a full NMC PIN to practice in the UK as we have paused our OSCE programme indefinitely. V300 Non-Medical Prescribing qualification or willingness to work towards Experience of the use of clinical systems Demonstrate initiative to proactively problem solve with a solution focus
