Community Staff Nurse (CAU/Frailty SDEC)

Within the Community Services Division of Wrightington Wigan and Leigh NHS Foundation Trust, The Frailty SDEC supports individuals who require both short term medical interventions and a period of bed-based, Nurse & Therapy led rehabilitation. It will support individuals to achieve specific goals which enable safe discharge back into the community or their own home.

This is a permanent full time post (37.5 hours). Frailty SDEC is operational 24/7/365 so shifts will be day or night.

Please note NMC registration is a requirement of this role.

Main duties of the job

Under the supervision of the ward manager, the nurse is responsible for the provision, organisation and direction of nursing care of the patients in his/her care. To communicate effectively with all members of the multidisciplinary team to ensure safe and high quality care is provided to all patients.

Job responsibilities

To provide the highest standards of care to our patients. Performs well within a multi-disciplinary team. Upholds NMC Code of Conduct. Adheres to Trust policies and procedures. Accountable for nursing practice and takes every opportunity to sustain and improve his/her knowledge and professional competence. Participates in the assessing, planning, implementing and evaluating programmes of care relevant to patients within sphere of responsibility and under supervision. To be sensitive to the needs of patients in relation to courtesy, privacy and dignity. To be familiar and adhere to Trust and Divisional objectives, policies, guidelines and current legislation. To complete patient documentation and undertake record keeping in a timely manner, ensuring that all entries are accurate and legible and that information systems are maintained. To provide information as required, including notification of adverse events and accidents to patients, staff and visitors. In conjunction with senior nursing staff and specialist nurses participate in the setting/monitoring and evaluation of standards of care to patients. To communicate effectively with all members of the multi-disciplinary team regarding patient care. To assist with the investigation and timely resolution of complaints and PALS enquiries.

Qualifications

Essential

NMC registration.

Desirable

Teaching and Assessing Course. Degree /Diploma Studies. Specialist Skills Course.

Experience

Essential

Experience in the relevant speciality of which you are applying. Experience of working independently, and as a member of a team.

Desirable

Post qualification experience. Previous experience in caring environment. Experience of Audit Research. Experience of Clinical Supervision.

Skills

Essential

Excellent verbal, written and interpersonal communication skills. Ability to maintain and improve standards of care. Ability to challenge unacceptable behaviour or practice in a professional context.

Desirable

Basic Computer Skills.

Knowledge

Essential

Up to date knowledge of current clinical and professional issues. Knowledge of evidence based practice. Understanding of research/audit.

Desirable

Understanding of resource management. Understanding of education/student requirements.

Additional

Essential

Flexible and positive approach to work. Speak English to an appropriate standard relevant to their role, i.e. with confidence and accuracy, using correct sentence structures and vocabulary, and without hesitation.
