An exciting opportunity has arisen for a highly motivated individual to join our Community Nursing Team covering the Central locality in the provision of planned care to patients in the community. Main Duties of the Job
You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for ongoing professional development. We also actively encourage newly qualified nurses to join our team. In return, we offer a preceptorship programme and support professional development in a nurturing environment, allowing you to reach your full potential. The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realize their potential. We are committed to clinical support, CPD & training. The post is covering 7 days, 8 am to 6 pm service. Person Specification
Qualifications
RGN NMC Registered Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes Experience
Experience of working in a multi-disciplinary / multi-agency health/social care environment. Experience and knowledge of basic administration systems including Microsoft Office Experience of successful partnership working arrangements with other services Previous community experience Skills
I.T. skills or willingness to undertake RIO Training Up to date with Mandatory Training Uses evidence-based practice Works within own defined competencies Demonstrates tact and diplomacy Well refined communication (verbal and written), interpersonal skills – ability to gain rapport at all levels Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well-informed manner Must be able to meet the transport requirements of the post You must have appropriate UK professional registration.
