We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.

This post is fixed term for 52 weeks.

We offer a relocation package of up to £8,000

The relocation package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership. Terms and conditions apply.

Main duties of the job

Our care community teams are based in The Fountains Health Center covering the surrounding geographic area. It's important that you can drive and have access to your own vehicle for work. Mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse, the successful candidate will undertake duties involving the delivery of high-quality nursing services and holistic assessments of patients, ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
