Our care community teams are based in The Fountains Health Center covering the surrounding geographic area. It's important that you can drive and have access to your own vehicle for work. Mileage expenses are reimbursed in addition to your hourly rate. As a Community Staff Nurse, the successful candidate will undertake duties involving the delivery of high-quality nursing services and the holistic assessments of patients, ensuring a person-centred approach. You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings. CWP provides health and care services for local people, including mental health, learning disability, community physical health, and all-age disability care - including the provision of three GP surgeries. Our aim is to help people to be the best they can be, adopting a compassionate, person-centred approach to everything we do. We were recently rated as Outstanding for Caring by the Care Quality Commission with a Good rating overall. We offer a variety of roles at all levels, so whether you are just starting out in your career or looking to use your skills and experience in a new role, CWP is the right employer for you. CWP is committed to making our recruitment practices as inclusive as possible for everyone, promoting equality and diversity, and developing a culture that values differences. To support this, we offer up to three weeks induction, dependent on role, with our Prepare to Care programme for all new starters. This programme welcomes you to CWP and provides you with information and training to support you in your role. We welcome applications from people who have direct experience of accessing our services. Do you have the skills to work in community physical health? Are you interested in working for a dynamic and innovative trust that provides person-centred health care to our local community patients? We'd love you to join our team. We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients. Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing. This post is fixed term for 52 weeks. We offer a relocation package of up to £8,000. This package is for anyone who would need to relocate in order to take up employment with Cheshire and Wirral Partnership. Terms and conditions apply.
