The Community Nursing Service within NorthEast Lincolnshire provides nursing care and support to patients, families, and carers in a variety of community settings, supporting patients to remain in their own homes, preventing hospital admission, facilitating early hospital discharges, and managing end-of-life care.

Our Community Nursing staff work closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses, and other healthcare staff to provide a quality service personalized to meet individual needs. The service supports and assists patients to manage ill health and long-term conditions, including elements of disability, thus allowing them to have the best possible quality of life while maintaining dignity and respect.

Main duties of the job Job title: Community Staff Nurse

1 X 37.5 hours Permanent

1 X 32 hours Fixed Term (Jan 2026)

Based: Pilgrim Primary Care Centre, Immingham

You will be required to work a rota pattern which includes weekends and Bank Holidays.

Salary Range 5: £29,734 - £34,474 per annum.

We are looking to appoint a qualified Registered Adult Nurse 1st Level with previous community experience and additional post-registration nursing qualifications (e.g., Tissue Viability, palliative care, nurse prescriber, etc.).

You will have the ability to maintain clear and accurate records using information technology and will have a proven ability to work on your own initiative as well as part of a team. The post involves the day-to-day management of an unpredictable caseload, some of which will be complex in nature, and therefore effective organizational and prioritization skills are essential.

The post requires you to transport yourself and equipment across locations within North East Lincolnshire and to deadlines. Therefore, a flexible approach to your working arrangements is essential.

Experience

Experience of providing teaching and education to others

Community Nursing Experience

Mentorship/Teaching Experience

Knowledge and Skills

Basic computer skills

Knowledge and application of current best practice in Nursing

Ability to communicate information to patients with varying needs, e.g., hearing impaired, learning difficulty, etc.

Ability to deal with conflict

Supervisory experience
