Are you an experienced Nurse who is interested in a community role, visiting and supporting patients with long-term conditions in their own homes? If so, this role could be for you. This opportunity is ideal for a proactive and enthusiastic nurse to support the Community Matron to manage the defined Matron caseload, and as part of the integrated team, you will maximize each other's skills and knowledge for the patients' benefit. The current postholder is retiring, and therefore this post would not be available until 27th December 2024. Main Duties of the Job

You will be expected to support holistic and clinical assessments to meet the patients' needs, providing education regarding how to manage their long-term condition while also working alongside a team of other professionals (District Nurses, Community Therapists, Integrated Care Coordinators, Health and Wellbeing Support Workers, etc.) within Fylde Enhanced Primary Care Team. The role will include building good relationships and working closely with GPs to ensure seamless, effective, and timely care. Support the Community Matron with the assessment, planning, implementation, and evaluation of the individual requirements of patients and carers on the Community Matron caseload. Provide a high standard of evidence-based nursing care to patients in the Community. Use a high level of communication skills with patients and their families, including those with English as a second language or learning disabilities, to communicate condition-related information. Qualifications

Registered General Nurse with NMC Current registration. Post-registration training in specialty area of Long-term conditions (i.e., COPD, diabetes, heart disease, asthma). Training in risk management. Experience

12 months community experience. Experience in Long-term conditions. Postgraduate experience working with a multidisciplinary team. Experience of working with patients with a variety of chronic disabling or life-limiting illnesses. Experience of delivering specialty training to colleagues in health and/or other agencies. Knowledge of current best practice. Ability to apply knowledge and principles of clinical governance. Previous use of computer-based data collection systems. Skills

Appreciation of research & evidence-based effectiveness. Able to relate theory to clinical situations. Able to plan and carry out assessment and intervention. Ability to make independent clinical and professional decisions, taking into consideration Trust and departmental policies and procedures. Ability to identify priorities, managing time, and meeting deadlines. Sound planning and organizational skills (clinical and non-clinical). Developed moving and handling skills. Implement advanced judgment and analytical skills. Ability to work alone as well as part of a team. Public speaking/presentation skills. Ability to work under pressure and recognize when support is required. Flexible, quick-thinking, resourceful, and able to accept advice and act on it. Able to negotiate and non-judgmental. Motivated, reliable, and approachable. Full UK driving license. Knowledge of current NHS trends which influence service provision. An understanding of current primary care organizational issues.
