Community Nurses make a difference every day to the lives of the people they visit at home. We are proud of our Community Nurses who provide increasingly complex care for patients and support for family members to encourage patients to receive the care they need in their own home.

We are looking for experienced Registered Nurses with a passion for high-quality nursing care and experience in either acute medicine, Emergency Care, or community nursing. The post holder will provide acute nursing assessment & interventions in patients’ own homes and will ensure that patients are referred onwards as appropriate to community services once their care episode is complete.

Working Life

As a Community Nurse, you'll:

Assess the healthcare needs of patients and families Monitor the quality of care they're receiving Be professionally accountable for the delivery of care Utilise technology to enable you to update records as you go

Your patients could be any age, but they'll often be elderly, while others may have been recently discharged from hospital, be terminally ill, or have physical disabilities.

The service aims to provide an excellent standard of care that supports patients to remain in their home environment. In Northamptonshire, you will work as part of a multi-disciplinary team to support the service and focus on delivering quality outcomes and improving the patient experience. You'll play a vital role in keeping hospital admissions and re-admissions to a minimum and ensuring that patients can return to their own homes as soon as possible.

Must Have Skills

You will need to be adaptable and resourceful. You'll need to be well-organised, confident, and able to cope in diverse situations to deliver patient care. Leadership and management skills are also important, as with all nursing roles; communication skills, problem-solving, and good judgment are crucial.

You will be part of a caring team and be able to work flexibly with minimum supervision, with the ability to prioritise work based on patient need.
