The Northwich Care Community has a post for a Community Staff Nurse for 37.5 hours per week based with Firdale DN team - we are looking for the right Community Staff Nurse to join our team! This post requires effective communication and interpersonal skills and the ability to be adaptable, reliable and approachable. Our Community Nurse Teams are responsible for the assessment of nursing needs, implementation and evaluation of care/treatment, and the delivery of high-quality holistic care within patients' own homes, residential care, and within our catheter and leg ulcer clinics. You will be expected to support your peers, the team leader, and the organization in promoting the Trust's values and behaviors. The hours of work are between 08:00 and 17:00, over 7 days. You will need to be able to work independently, possess good time management and organizational skills, and be expected to be innovative and flexible to work in other areas if required to meet service needs. Please note that while your main base will be in Northwich with the Firdale Team, you may be required to work across the footprint of Central Cheshire Integrated Care Partnership. We are looking for a Band 5 staff nurse to support the caseload holders in delivering high-quality, efficient, and safe care. We are looking for nurses who are newly qualified, community experienced, or have no previous experience; for our team, it is about the individual. We are looking for an enthusiastic person who possesses both clinical and digital skills to support in the promotion of quality improvement for a local population. Main duties will involve wound management, chronic condition management, catheter management, and palliative support and end-of-life care. Our patient groups are varied and often complex. Job Responsibilities

To work under the direction of the Community Nurse Team leader and be responsible for the assessment and management of the health needs of an identified population. To support and maintain patients in their own preferred place of care, maximizing their independence and optimizing their quality of life. To liaise with all healthcare professionals and other agencies to ensure appropriate and effective service delivery. To assess, provide, and review holistic nursing care, advice, and information to patients and carers within the home environment or appropriate care setting. To undertake initial assessments on receipt of specific referrals to the caseload. To provide skilled nursing care, including wound care, leg ulcer assessment and management, administration of medication, and catheter care. To promote clinical excellence and evidence-based approach to care by complying with Trust policies and maintaining clinical competencies. To participate in health education of individuals and the community, promoting self-care. To accurately record all patient contacts on Emis web and Trust documentation. To ensure that equipment and aids necessary for effective care meet the needs of individual patients. To participate in the delivery of care involving complex needs and support carers and relatives in distressing circumstances. To manage and assess patients' discharge potential and needs from the caseload. To maintain accurate personal and professional records adhering to MCHFT and Nursing and Midwifery Council policy and guidelines. To report accidents/incidents of patients or self which occur whilst on duty. To maintain all aspects of MCHFT Integrated Governance Strategy and requirements. To act at all times in a manner which illustrates respect for privacy, dignity, and confidentiality. Person Specification
