Would you like to work with a forward-thinking and innovative team in Community Nursing? If so, Portsmouth Community Nursing Team is recruiting. We are seeking a Band 5 RN who is keen to work & develop their skills within the community setting. You will work within a Primary Care setting, led by one of our Community Matrons and will work alongside other professionals such as GPs & Social Services. You will be an autonomous practitioner and have excellent communication and interpersonal skills. You will work to a high professional standard, enabling us as a team to provide high-quality, person-centred care. The Community Nursing service provides nursing care to patients at home across a 24-hour period with a Day Team (0830-1630) and OOH's team (Twilight 1400-2200, Nights 2130-0830) seven days a week, 365 days a year. Successful applicants will have good communication and interpersonal skills and will need to be flexible and adaptable. Community experience is desirable although not essential as we have an innovative competency-based approach to training and development to support you to maximize your career potential. In return for your skills and commitment, we will offer you regular supervision, internal and external training opportunities, and regular contact with other professionals across the area. Must be a car driver with a valid driving licence or have access to transport with appropriate business insurance in order to travel throughout the Trust, to meet the needs of the service. Main duties of the job

The post holder will take responsibility for managing, coordinating patient care and utilizing other services. You will participate in inter-professional working and the development, implementation, and evaluation of the delivery of health care services. Safely manage high-risk areas including: Lone working in the community Patient handling in challenging environments where the principles of best practice regarding patient handling may be compromised. Driving self Handling medical sharps and adhering to infection control policy Handling body fluids and other hazardous materials You will have responsibility for providing the highest standards of patient care through ongoing assessment, planning, implementation, and evaluation of programmes of care. Working with the District Nursing team and other agencies, you will use your advanced person-centred assessment skills for the health and well-being of patients with complex needs. About us

Qualifications

Registered Nurse Experience

To be able to manage the team and caseload in the absence of the team leader. Excellent communication skills, written and verbal. Ability to work independently. Leadership including problem-solving, prioritizing, and delegating appropriately. Time management. Ability to inspire confidence. Advanced person-centred assessment for the health and well-being of patients with complex needs. Planning, delivering, and evaluating care to address people's changing health needs. Identifying and managing risk to the Organisation, Service, and individual, staff and patient. Able to develop and sustain relationships across professional boundaries. Ability to secure the cooperation of colleagues at all levels across agencies. Ability to work as part of a close-knit team. Self-motivated, self-starter, able to organise a large and varied work programme. Being flexible and adaptable at work in order to meet competing priorities. Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being. Knowledge of the pathology of disease and illness processes. Multi-agency services available for adults and processes for referral. Additional Criteria

Knowledge of National agendas and the effect on practice e.g., NSF's including Palliative Care, and the Public Health Agenda. Be able to demonstrate an understanding of the principles of equality, diversity, and inclusion and how they apply to staff and patients in a healthcare setting. Able to demonstrate personal commitment to challenging discrimination and promoting equalities, at an appropriate level for the post. Is able to work legally in the UK. Is able to work with children and vulnerable adults. Ability to travel daily for meetings across a range of sites in Southampton/Portsmouth/Hampshire. For posts which require a professional registration, postholders hold a valid up-to-date registration with their professional body. For posts where postholders are required to drive as part of their role, they hold a valid full driving licence which enables them to drive in the UK. Disclosure and Barring Service Check
