Job Summary Work as a member of the School Nursing service within a designated locality supporting the School Nurse Team to deliver on public health priorities in Somerset to school-age children in line with current national Public Health Agendas, working to National and Local objectives as defined by Service Specifications under the supervision and guidance of the Qualified School Nurse. Work with vulnerable children and families in line with national and local policies relating to safeguarding children and provide specialist support and advice to children, families, and schools regarding the health and wellbeing needs of children. Work in partnership with other agencies, particularly the Local Authority, including Youth Workers. Support the delivery of programmes of intervention for a caseload made up of schools and individual children and families. Deliver National and local programmes on an individual, group, and community level. Supervise and guide the School Nurse team assistant practitioners and administrator. Key Responsibilities under supervision and guidance of the Qualified School Nurse Participate in the delivery of the National Child Measurement Programme. Identify need through Health Needs Assessment and provide personalised advice, support, and interventions to young people regarding health and social issues including relationships, emotional health and wellbeing, and risk-taking behaviours. Complete contemporaneous electronic record keeping including report writing and client records in line with the Organisation's policies and NMC code of Practice. Participate in the promotion and delivery of young people's clinics in partnership with the educational and community establishments to promote general health and wellbeing of children and young people. Work collaboratively as a member of a multi-disciplinary team with other agencies to maximise the School Nursing service contribution to health improvements in the local community. Facilitate and deliver targeted groups in a variety of settings. Contribute to the delivery of health and social learning programmes for young people with identified needs. Contribute to the delivery of care programmes for vulnerable children in conjunction with the school nursing team, families, school, and other professionals. Liaise with Health Visiting teams regarding children who may have additional needs relating to transition between pre-school and school and support the transition. Understand, uphold, and promote the aims of the councils' equality, diversity and inclusion policies; health, safety and wellbeing of self and others; and Organisational values in everything you do. The postholder will complete a period of induction to achieve the competences required to undertake the role. Contacts & Relationships School Nurse teams across Somerset. External relationships with other agencies and Acute NHS Trusts. Allied partnerships, e.g. Drug & Alcohol Action Team. Communicate in person, face to face, in writing and, where appropriate, electronically with all the above contacts. Establish clear lines of communication between all members of multi-agency and professional teams, working collaboratively with all relevant healthcare professionals and agencies. Vary the style and level of communication with individuals, families, and carers to meet the differing levels of understanding. Actively involve young people (where appropriate) in decisions about their own healthcare. Attend and contribute to Area team meetings monthly to facilitate effective communication within children and young people's services. Resources Participate in the supervision, teaching, and provision of experiential learning opportunities for nursing students and other personnel as appropriate. Participate in the appraisal programme and undertake appraisals with skill mix staff as required. Somerset Council is subject to Part 7 of the Immigration Act (2016) and therefore the ability to speak fluent English will be an essential requirement for customer-facing roles. This job requires a criminal background check (DBS) via the disclosure procedure.
