Job summary Dorset HealthCare are seeking a Community Staff Nurse to join our vibrant and innovative Integrated Care Team in Purbeck, in the District and Community Nurse team in Wareham. The post is for a fixed term of 12 months to cover maternity leave. You would work alongside our experienced Community Nurse Teams helping to provide care needs to a high-quality standard of care for patients in their own homes. We can be flexible with shift patterns as long as the service needs are met between hours of 08:00-20:00 with some attractive weekend enhanced pay. We offer a structured induction programme, flexible working patterns and pastoral support. There are excellent learning and development opportunities, both in-house and through our links with local universities, plus the option of a preceptorship programme for newly qualified nurses to boost your skill development. This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only. Please obtain permission to be released on a secondment before submitting an application. Applications from candidates that require current Skilled worker sponsorship to work in the UK are unfortunately ineligible to apply for this position. Main duties of the job Provide high-quality person-centred approach to care delivery which considers people's safety, privacy, and dignity while promoting and embodying the Trust's values. Undertake assessment of patients, care needs, and develop programmes of care as part of the wider multi-disciplinary care team. Manage acute and chronic nursing needs, developing strong therapeutic relationships with patients. Work closely with community partners to achieve the best possible health outcomes. Identify care needs, monitor patients' conditions, record observations, and help develop wellbeing care plans. Support people to maintain their wellbeing and independence, helping to avoid unnecessary admission to hospital, and deliver exceptional end-of-life care including symptom management and assistance in identifying and achieving people's preferences.
