This is a part-time Community Nursing position designed to contribute to avoiding hospital admission and maintaining patients' independence in their own homes throughout the Waterlooville locality. This is achieved through routine visits and responding to Urgent Crisis Response calls, which are a key aspect of the role. The UCR process involves working alongside SCAS, offering acute visits with other members of the MDT, and using community-based skill sets to reduce the need for an ambulance and prevent patient admission. This post offers the opportunity to work closely with all members of the ICT in primary, secondary care, and adult services, as well as care home staff. The successful candidate will actively participate in ICT/MDT meetings, coordinating a plan of care and working autonomously as a registered practitioner. They will be involved in care planning and identifying person-centred care delivered in the home setting. Excellent communication skills and the ability to work collaboratively with the larger multidisciplinary team are essential. The job involves a significant amount of driving; therefore, you must be a car owner and have access to a vehicle for work with the correct level of insurance. Main Duties of the Job
Working within professional guidelines and accountability, the post-holder will manage a defined community caseload (via Nursing, Physiotherapy, or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team. The post-holder will manage, coordinate, and delegate work, ensuring referrals are dealt with according to clinical need. Holistic assessments and care plans must be recorded in line with Trust and professional record-keeping standards, ensuring clinical care meets acceptable standards of patient safety and quality outcomes. The post-holder will work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admissions to an acute hospital and support timely discharge into the community. The post-holder will also be responsible for re-enablement, rehabilitation, health protection, and promotion programmes that improve and reduce inequalities. About Us
