The Community Support Service supports people to regain their independence through reablement, offering a period of short-term recovery. We are a CQC registered service that is rated overall 'Outstanding'. Our reablement team works with individuals to increase their ability to look after themselves following illness or injury, rebuilding the skills and confidence to help them live as independently as possible. We work closely with colleagues and partners in health and the voluntary sector to ensure an integrated and collaborative approach to our service. Unlike traditional home care, we support and encourage people to carry out everyday activities themselves. We collaborate with our occupational therapy and social work colleagues to assist individuals coming home from the hospital and in the community to maintain their independence. The Community Support Service is looking for flexible, motivated, and enthusiastic individuals to join our brilliant team of Assessment Officers. The role is for 37 hours per week on a rota basis, including weekends and bank holidays. The Assessment Officer role is crucial to maximizing the independence of our customers. They undertake a strength-based assessment where the customer sets their own goals and agrees on a plan to meet these with support, help, and guidance from a dedicated team of community support workers. This can include identifying support around medication, self-care, confidence building, and signposting to community resources. You will need to have a full driving license and access to a car when at work. We are deeply committed to providing training and development.
