As a Clinical Support Worker, you will need to be flexible and hardworking, expected to see patients in their own home environment under minimal supervision within your competencies and adhering to SWFT's Lone Working Policy.

You will have at least 1 year of care experience, preferably in the community, and possess some knowledge and skills in managing the care of long-term conditions, wound and pressure ulcer care, and end-of-life patients. Enthusiasm and excellent communication skills, along with being a good team player, are essential attributes for this role.

Main duties of the job The Band 3 Community Support Worker will undertake various clinical skills, such as complex wound care, care of chronic lower limb disorders, and application of specialist compression therapy, under the direction of registered staff while working as part of the multi-disciplinary team.

As a Community Support Worker (CSW), you will make a valuable contribution to the delivery of high-quality healthcare, supporting registered professionals in delivering safe, effective, compassionate patient care. Clinical Practice

Accurately and promptly carry out the instructions of the registered professional accountable for care delivery.

Assist in the supervision of other healthcare and support staff as directed.

Carry out duties specified within the Trust's approved core task list for Clinical Support Workers/Healthcare Assistants without direct supervision.

Report any patient concerns or observed changes in patient condition to the registered professional.

Provide and deliver a high standard of individualized care under the direction of the registered professional.

Document all care delivered to patients, countersigned by a registered professional, maintaining confidentiality.

Assist in preparing patients for treatment, investigations, or procedures as directed.

Maintain a clean and safe clinical area, adhering to infection control procedures.

Chaperone or escort patients as needed under the direction of the registered professional.

Support relatives with compassion and empathy under the guidance of a registered professional.

Assist with the discharge process for patients as directed.

Communications and Interpersonal Relationships

Welcome patients, relatives, and visitors to the clinical area, providing assistance and information as needed.

Introduce yourself in line with the Trust's privacy and dignity promises.

Answer the telephone courteously and pass on inquiries to the registered professional.

Communicate effectively within the multi-disciplinary team to ensure coordinated patient care.

Contribute to reporting, handover, and documentation of patient care as delegated by a registered professional.

Report incidents, accidents, and complaints per Trust Policy to the senior professional on duty.

Use Trust information support systems and other computer systems as required.

Team Working

Ensure the safe keeping of patients' property in accordance with Trust policy.

Promote a professional and tidy environment, keeping storage areas safe and clean.

Share responsibility for the care and maintenance of all equipment in the clinical area.

Ensure all waste and soiled linen is disposed of per Trust policy.

Participate in audits under the direction of the registered professional.

Clean and check beds, bed spaces, and lockers in the patient area following discharge.

Ensure deliveries are stored correctly and stock levels are maintained.

Support the housekeeper with general housekeeping duties, reporting essential cleaning needs.

Education, Training and Personal Development

Attend mandatory training sessions, including infection control, resuscitation, and manual handling.

Complete the Trust's Clinical Support Worker/Healthcare Assistant Induction programme.

Participate in appraisals and personal reviews to achieve set objectives.

Undertake necessary training for post development and departmental relevance.

Maintain up-to-date knowledge of changes in policies and procedures relevant to the post.

Person Specification Qualifications

GCSE level A - C English and Maths or equivalent.

Minimum NVQ/Diploma Level 2, willing to work to Level 3 or equivalent higher apprenticeship in healthcare.

NVQ/Diploma Level 3 or equivalent higher apprenticeship in healthcare.

Experience

Experience delivering care in a healthcare or people-centred environment.

Experience working as part of a team.

Skills

Evidence of competence in care skills.

Good written, verbal, and non-verbal communication skills.

Able to present information accurately and logically in written and oral forms.

Ability to organize and prioritize workload.
