Key Tasks and Responsibilities of the Post You will work as part of a Multi Disciplinary Team in accordance with Livewell policies in order to enable individuals to access mental health specialist support when required, in line with Recovery and Trauma Informed Principles. You will be involved in the co-production of holistic, evidenced-based care plans in line with the Personalised Care Framework. Under the guidance of a registered professional, you will provide holistic, evidenced-based, recovery-focused, and trauma-informed care to individuals and groups, and support other agencies to contribute to this care, maintaining accurate clinical records. You will provide interventions to a variety of people and their families known to the CMHT enhanced team, to promote recovery and resilience. You will maintain links with people admitted to hospital to ensure they maintain connections to their community where possible. You will attend and contribute to multi-agency forums regarding service provision and the individual care of service users. You will practice within the legal and ethical framework set out by mental health-related legislation to ensure that service users' interests and well-being are prioritized. You will work collaboratively with other statutory and VCSE organisations in line with relevant policies. You will liaise, advise, develop, and maintain good relationships with service users, carers, relatives, and fellow health and social care professionals involved in the delivery of services. On a day-to-day basis, you will be supported by a shift lead who provides appropriate supervision and advice to others. You will contribute to maintaining a psychologically safe and positive learning environment for all staff, providing clinical advice and guidance, and line management for junior staff and students as required, while participating in training programs designed for all staff groups. You will take individual accountability for your practice, maintaining your own professional development, participating in appraisal, performance review, and all activities aimed at keeping professional skills updated. You will contribute to monitoring standards of practice within the team through the use of clinical reporting systems and participation in the services clinical audit programme, which will include annual Personalised Care Framework reviews, medication, and Health Records audits. You will contribute to the collation of evidence within the team in support of standards for better health, improving quality. You will actively support the team through changes in organisation and service delivery through the implementation of the Personalised Care Framework. You will actively encourage people and their carers to be involved in service development and seek information and advice from key individuals when outside of your area of expertise. You will work collaboratively with the IPS, providing information and support to enable service users to engage in meeting education, employment, and vocational needs, which may include arranging initial contact and attending with the service user as required.
