Working as a locum Community/District Nurse provides opportunities for continuous professional development. Main duties: Holistically assess, plan, deliver and evaluate individual programmes of care in partnership with patients, carers and other health and social care professionals. Be accountable for an identified caseload. Refer patients to alternative services as determined by their clinical need. Communicate effectively with clients/carers and colleagues to agree treatment goals. Maximise rehabilitation potential. Ensure understanding of conditions and treatment. Manage patient expectations regarding the service. Deliver a range of clinical skills, commensurate with the clinical competencies required for a Band 5 Community Staff Nurse. Recognise and work within own level of competence and seek advice where appropriate. Utilise clinical expertise in identifying patients’ needs and ensure acute and chronic phases are responded to appropriately. Discharge dates are met wherever possible. Understand and manage risk within patients’ homes and other settings. Liaise with acute partner services to ensure patients are redirected to the appropriate service if required.
