We are seeking a highly skilled nurse, social worker or Occupational Therapist to join our Bed Management team in the role of Complex Case Manager.

Main duties of the job The successful candidate will be based at Sceptre Point, Sceptre Way, Walton Summit, and will be responsible for providing strong, visible day to day clinical leadership skills. The post holder will actively case management a cohort of service users with complex needs, co-ordinating and progressing discharges.

The post holder will be required to maintain constructive relationships with a range of stakeholders. The Complex Case Manager role will provide clinical support to Flow leads/OAP practitioners to maximise opportunities to enable safe discharge from hospital. The post holder will focus and lead on patients with complex needs/long hospital admission and will work with agencies to progress discharges.

About us Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.

The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.

Person Specification Qualifications Essential Appropriate professional qualification RMN/RGN/RNLD/ Social Worker/ Occupational Therapist

Desirable Management qualification and/or experience Evidence of post graduate level study to masters level and or working towards

Knowledge Essential Importance of Equality, Diversity and Rights. Detailed knowledge of professional accountability, and impact on NHS issues.

Skills Essential Ability to lead meetings and discussions with senior staff. Ability to handle complex and difficult situations competently with patients, carers, staff and external agencies. Extensive knowledge of community and inpatient services Detailed knowledge of national policies and strategies relating to patient flow and the skills to lead and implement these.

Experience Essential Experience of caseload management Experience of managing complex cases. Co-ordinating and progressing discharges
