You'll be responsible for supporting the ward staff with complex discharges, taking the lead in discharge arrangements for individual patients where appropriate, liaising effectively with all members of the Multi-Disciplinary Teams in Community settings. £30 p/h Umbrella (potentially negotiable depending on experience) Working Hours:
Monday-Friday, 9am-5pm Contract:
