Based in the colourful and thriving Borough of Hammersmith and Fulham in West London, we are looking for a dynamic and skilled colleague to provide consultant input to Meridian Ward, an 18-bedded in-patient ward for patients aged over 70 with a functional mental health illness within the Older People’s Mental Health Service (OPMHS). The post-holder will be working alongside the Specialist Older Adult Mental Health Service (SOAMHS) and Cognitive Impairment and Dementia Service (CIDS) to ensure seamless and holistic care of patients. It is currently a specialist commissioned service with a highly skilled and synergistic multidisciplinary team. The Care Quality Commission recognised the CIDS as “Outstanding” for patient care and “Good” for all other service domains. We are looking for a colleague with experience of working in older adult mental health services who is an excellent clinician, enthusiastic leader and mentor and who is passionate about continuous service improvement. You should have good communication and interpersonal skills and be able to demonstrate an ability to work effectively and flexibly with a multi professional team. Main duties of the job

It is expected that the medical team members will take a lead role in providing diagnosis and management of new patients after non-medical triage, requesting and interpreting investigations, and discussing with patients and their carers post-diagnostically. The post holder will take a leading role in developing the service, supervising junior colleagues and providing lead medical input to a multi-disciplinary team. The post holder will have excellent clinical skills in assessment and treatment of older adults with mental health issues. As a senior clinician, the post holder will have an integral role in the performance of the team, further development of the service, and the development of the clinical pathways and priorities of the service line. They will also be expected to provide clinical and professional leadership within the post and to take part in management activities and governance processes related to the post or the wider service within which it is embedded. Qualifications

MBBS or equivalent medical qualification Qualification or higher degree in medical education, clinical research or management Additional clinical qualifications Fully registered with the GMC with a licence to practice at the time of appointment GMC Specialist Register or within 6 months Approved Clinician status OR able to achieve within 3 months of appointment GMC Specialist Register (old age psychiatry or dual training in both adult and old age) or within six months Good standing with GMC with respect to warning and conditions on practice Approved under S12 OR able to achieve within 3 months of appointment Academic skills and lifelong learning

Excellent written and oral communication skills Able to deliver undergraduate or postgraduate teaching and training Ability to work in and lead team Able to manage clinical complexity and uncertainty Makes decisions based on evidence and experience including the contribution of others Demonstrate commitment to shared leadership & collaborative working to deliver improvement Has actively participated in clinical audit and quality improvement programmes Experienced in clinical research and / or service evaluation Evidence of achievement in education, research, audit or service improvement e.g. prizes or awards Evidence of improving patient outcomes through service change or audit Clinical knowledge
