Care co-ordinators help to co-ordinate and navigate care across the health and care system, helping people make the right connections, with the right teams at the right time. They support people to become more active in their own health and care and are skilled in assessing people's changing needs. Care co-ordinators are effective in bringing together multidisciplinary teams to support people's complex health and care needs. They can be an effective intervention in supporting people to stay well, particularly those with long-term conditions, multiple long-term conditions, and people living with or at risk of frailty. BGPA Department Information BGPA is a federation of all 43 Bromley practices working collaboratively to enhance the health and wellbeing of Bromley residents, covering over 350,000 patients, 100% of the Bromley population. Job responsibilities

Job Purpose: To contribute to the Primary Care Network achieving its goal of improving patient care amongst its member practices. Key Relationships: PCN Directors, PCN Network Manager, PCN Practice Managers, GPs and other Practice staff, BGPA management, other local health and social care organisations, patients, and carers. Job Summary: Provide administrative support for Clinical Directors; Carry out administrative work to support patient clinics such as covid vaccination clinics and nurse clinics including setting up clinics, booking staff, arranging appointments, liaising with other staff and patients when necessary; Liaise with ARRS staff regarding PCN and practice indicators and targets; Monitor Practices performance against contract targets and support work towards achieving those targets and feed back to Clinical Directors; Carry out administrative work for meetings including taking minutes and actioning action points as required; Carry out searches on the clinical computer system and report activity data to the Clinical Directors and Network Manager; Utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care; Provide co-ordination and navigation for people and their carers across health and care services working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals; Assist the PCN Clinical Directors and Network Manager with regard to the Network Contract Directed Enhanced Service; Support the other care co-ordinators within the PCN and help develop their role; Organise PCN publicity and events when required; Maintain good communication with PCN practices; Participate in training, as appropriate; Work within organisation policies, procedures, and guidelines; Any other duties considered appropriate to the post. Experience of working directly in a care coordinator role, adult health and social care, learning support or public health / health improvement Experience of working with a framework of confidentiality Experience of working in health, social care, and other support roles in direct contact with people, families, or carers (in a paid or voluntary capacity) Experience or training in personalised care and support planning Experience of data collection and using tools to measure the impact of services Ability to organise, plan, and prioritise on own initiative, including when under pressure and meeting deadlines High level of written and verbal communication skills Ability to work flexibly and enthusiastically within a team or on own initiative Ability to maintain effective working relationships and to promote collaborative practice with all colleagues Ability to demonstrate personal accountability, emotional resilience, and work well under pressure Ability to communicate effectively, both verbally and in writing, with people, their families, carers, community groups, partner agencies, and stakeholders Ability to work from an asset-based approach, building on existing community and personal assets Knowledge of, and ability to work to policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety Ability to actively listen, empathise with people, and provide personalised support in a non-judgemental way Commitment to reducing health inequalities and proactively working to reach people from diverse communities Skills and Knowledge
