The Credentialled Diabetes Educator will be a valued member of the Albury Wodonga Community Health Diabetes Service. The purpose of the role is to provide diabetes education, assessment, and intervention for patients and their families within the primary and community health service of AWH. The role includes expert level consultation and integrated care planning with internal and external service providers, as well as participation in diabetes education service planning and development for continual service improvement aligned with local population health needs and service context. Minimum Requirements : Relevant undergraduate qualification in a related discipline with Postgraduate qualification in Diabetes Education / Management Member of the Australian Diabetes Education Association Credentialled Diabetes Educator Current valid Driver's Licence Advanced current knowledge and skills in the clinical management of people with diabetes Knowledge and skills in contemporary service models and self-management approaches to chronic illness care Ability to plan, implement, and evaluate diabetes education and management programs Excellent interpersonal, communication (written and verbal), and presentation skills, with the ability to work as part of an interdisciplinary team. This is a fixed term part-time (16 hours per week) opportunity for a period of 12 months.
