Responsibilities: Respond to consumers in crisis and provide timely and appropriate levels of care, which may include but not limited to screens, collaborative phone contacts, and crisis sessions. Actively participate in staff meetings and provide updates and clinically relevant information to regular treatment team staff. Assist consumers to seamlessly integrate into regular outpatient services. Develop continuous supportive relationships with local and state hospitals, law enforcement, courts, schools, and social service agencies to coordinate a comprehensive continuum of care emergency services program. Assist in conducting public information programs for community partners. Perform intake assessments when not providing emergency services. Provide 72 hour follow up sessions to consumers placed in crisis services. Communicate and work closely with CSS, CBS, Crisis, TCM, and hospital liaisons. Participate in CBST Meetings and complete PRTF screens as requested. Develop and document diversion plans when consumers can be supported and treated in a less intensive environment than the hospital or structured placement. Maintain consumer service clinical records routinely and in a timely manner. Document all services provided in a clear and professional manner. Be aware of local community resources that may assist consumers in crisis and respond to community partners contacts for assistance. Sustain a prominent level of professional competence through continuing education and clinical supervision. Maintain all professional credentialing requirements. Adhere to the standards of ethics for mental health professionals. Follow all policies and procedures of the agency. Travel is required daily to engage consumers in the environment in which they need the most support. Provide other duties as assigned.
