Monday - Friday 09:00 - 17:30 Working Hours:
36 hours per week Assignment Period:
3-6 Months Ongoing Essentials: Shifts will be day, evening, and sleep overs. No waking night shifts. Must be able to work Sundays. Health & Social Care - Level 5 3 years management experience required for this role minimum. You will need to: Deliver excellent support to vulnerable adults with a Learning Disability, physical disability, Autism, Challenging Behaviour, Mental Health, or Dementia. Work in partnership with services, schools, and colleges on our transition groups for residents who are leaving children and young people's provisions and moving into adults' provisions. Assist our residents in identifying progression opportunities and providing ongoing transition support for those moving into college, employment, apprenticeships, or further learning. Help residents live the life they want by establishing positive relationships, supporting and enabling them to increase their confidence, realize their potential, and fully integrate them into all project activities. Explore services such as Independent Living establishments as required, and a Curriculum for Independent Living skills.
