The Patient Pathway team provides high-quality services consisting of managing outpatient waiting list performance, health records management, patient access to outpatient appointments, and information governance that support HDFT's short and long-term planning process and meet the needs of the Trust, Monitor, Department of Health, and Local Health Community. The team is responsible for: Maintaining Outpatient clinics on the Patient Administration System Managing the appointments system Preparing health records for outpatient clinics Providing reception cover for key desks across the Trust
