We are a dynamic and forward thinking team looking for a
band 6 nurse
to join our established team as a
Deputy Team Lead .
A great opportunity has arisen for an experienced and motivated registered nurse to lead one of our neighbourhood teams under the direction and leadership of the Deputy Locality Manager.
Your role would be varied and interesting carrying the responsibility for a patient caseload which involves the assessment, planning, implementation and evaluation of evidence based care to patients in their home environment. Previous community experience is essential.
The successful applicant must be someone with a strong commitment to working as part of a team, be approachable, a good communicator, have the ability to work autonomously and be receptive to change.
Main duties of the job This is an exciting time to join the community nursing service in West Herts. We are developing new community pathways which will support the delivery of more care in people’s homes, and to enable us to do this we have a range of new learning and development opportunities available through the CLCH Academy.

Person specification Education/Qualification
Registered Nurse with current UK NMC registration
Mentor/Preceptor/998 or equivalent
Specialist Nursing Practice (District Nursing) Degree or will undertake training
Nurse Prescribing or will undertake training
Evidence of personal development
Independent Prescriber Experience
Relevant courses in Long Term Conditions Management
Course of Top to Toe examination/diagnosis Experience
Experience
Minimum of 3 years’ experience at band 5 Community Nurse level
Management of a team
Management of change within a complex environment
Experience of teaching others
Proven clinical skill at Senior Community Nursing level
Experience of participating in investigation/management of complaints
Evidence of participating in clinical audits in the workplace
Knowledge of recent developments in NHS
Knowledge of Evidence based practice
Knowledge of NMC code of professional conduct
Other
Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010)
