The post holder will provide specialist tailored dietetic support for persons with obesity who have complex medical comorbidities and needs and are residents of the borough of Croydon. The post holder will: Be an integral part of a high-quality specialist multidisciplinary Community Diabetes Service for adults registered with a Croydon GP. Act as a link between primary and secondary care diabetes service. Be a source of specialist knowledge and support, working with community and acute health care professionals through one-to-ones, groups, and joint clinics. Support the nutritional management of diabetes and actively participate in dietetic student training and gestational diabetes education groups. Deliver diabetes education for persons with diabetes, for example, DESMOND, carb counting. Provide an equitable and inclusive service that is flexible and supports a person-centred approach.
