What you can add: The CSH Diabetes Specialist Nursing team provides specialist clinical advice and care, supporting the delivery of all commissioned activities to adult patients within the North West Surrey area and delivering an integrated model of diabetes services. The role of the Diabetes Specialist Nurse (DSN) service is to promote the health and wellbeing of people with diabetes enabling them to achieve optimum glycaemic control through informed self-management. The post holder will provide a link between GP and consultant-led specialist teams and will provide education and support both to people with diabetes and those who care for them. The post holder will be responsible for facilitating improved diabetes care through the provision of advisory services, including telephone, video, and face-to-face support within practices, clinics in an acute setting, nursing homes, and patients' own homes. The post holder will be responsible for contributing to the ongoing development of clinical practice, research, and standards of care within the service. The role will involve, but not be limited to you performing the above. What you'll need: Qualifications: First degree in Nursing or equivalent level of experience NMC Registered General Nurse Evidence of certified continual professional development within specialist area. Post-graduate certificate in diabetes Mentorship course. Experience: Minimum of five years experience as a registered nurse. Evidence of working in a multidisciplinary team. Experience of working with people with diabetes, both type 1 and type 2. Research or clinical audit experience. Community health care experience. Skills: Ability to clinically assess, deliver and evaluate high-quality care. Ability to work without direct supervision Able to reflect on own strengths and development needs Ability to work under pressure, balance multiple priorities and meet deadlines Ability to demonstrate use of evidence-based practice in line with clinical governance principles. Ability to develop new skills appropriate to the role. *Depending on the Trust, you may require variable additional certification and skills.
