Diabetes Specialist Nurse/ Intermediate Care Education

Our intermediate role bridges between the community diabetes services and hospital diabetes services of West Kent, focusing on education, diabetes technologies, supporting discharge, and reducing avoidable admissions. The ideal candidate would be an experienced diabetes specialist nurse, or diabetes practitioner currently working in primary/community care or a complex care nurse with diabetes responsibilities. This role requires the ability to work independently to a high standard, be highly self-motivated, organized, and enthusiastic about the delivery and organization of diabetes care between primary and secondary care. This role supports the smooth transition of patients to their own homes and nursing/residential homes that might need access to diabetes technology and education to support this transition. This role also provides priority reviews for type 2 patients that have had delayed community review. We are looking for an innovative, ambitious, and forward-thinking diabetes nurse to help us drive improvements within service delivery. A driving license is essential for this role. We are particularly interested to hear from Diabetes Specialist Nurses or Practice nurses with community and/or education experience. Main duties of the job

You will act as a specialist resource and provide specialist advice in relation to care of people with diabetes, providing education and training in various settings. Once your pre-employment checks have been completed, you will be contacted by Recruitment to arrange your start date, which will be in line with one of our Corporate Inductions. Following your Corporate Induction, you will be expected to attend the Trust's 2-week Induction Programme for RNs, NAs, RMs, ODPs, and Paramedics. The programme is full-time (please email mtw-tr.professionalstandards@nhs.net if this is problematic) and will involve travel to both Trust sites. The programme includes supernumerary shifts in the clinical area, classroom-based sessions, and e-learning. Please note that appointment to this post will be subject to a satisfactory Disclosure & Barring Service Check at Enhanced Level. Qualifications

Level 1 NMC Adult Registered Nurse Diabetes Diploma/Certificate Teaching qualification or equivalent expert experience Non-Medical Independent Prescriber or willingness to undertake within 12 months Mentorship qualification or willingness to undertake Counselling qualification or willingness to undertake Experience of managing diabetes in specialist clinics (renal, pregnancy/gestational/pump, YAC) Experience/Knowledge

Minimum 3 years post-registration experience (full-time hours) Extensive knowledge of diabetes with previous experience in a DSN role for a minimum of 12 months Ability to work independently and as part of a team A working knowledge of the management of Type 1 & Type 2 Diabetes Knowledge of diabetes specialist clinical treatments and technology Experience of clinical supervision and mentoring junior staff/nursing students Experience of teaching and presenting Skills

Excellent oral and written communication skills Good clinical assessment skills or willingness to develop these Good listening, observational, and interpretive skills Demonstrate effective teaching skills Excellent interpersonal skills Awareness of own limitations and when to seek advice from senior members of the team Experience of undertaking audits Disclosure and Barring Service Check
