We are looking for an ambitious, dynamic, motivated and enthusiastic Band 6 registered nurse qualified or with a specialist interest in Diabetes, to join our Community Nursing Service in Bedfordshire. This fixed term post has been created in order support the Community Nursing teams reduce and manage the volume of patients being visited for insulin management.

Main duties of the job

The primary focus of this post will be to support the education and promotion of self-management, including the review of current insulin regimes and possible onward referral for diabetes management.

You will be a strong leader, focused on achieving real improvements in outcomes for patients and will be confident in your ability to support, coach and develop patients and teams to deliver self-care in the management of diabetes. You will be innovative, forward thinking, self-driven with excellent communication and interpersonal skills.

You will take pride in your work and be highly motivated and dynamic, have a flexible approach and be able to embrace change. Being a collaborative worker and the ability to network and build relationships with patients and stakeholders is essential.

Education/Qualification/Training

Essential

Level 1 RN / BSc Hons Degree NMC registration DESMOND trained and accredited or willingness to undertake training and accreditation. Evidence of continuing professional development in Diabetes management

Desirable

Member of specialist clinical interest group Evidence of audit and data Analysis Counselling Skills Post registration qualification in teaching

Experience

Essential

Experience in managing diabetes patients. Experience of working in a multi-disciplinary team Knowledge of NMC code of conduct and / or guidelines of professional practice Commit to the principles of clinical governance and clinical supervision Ability to produce clear accurate written/ verbal records

Desirable

Experience of working in a community setting

Knowlege and Skills

Essential

Excellent communication skills both verbal and written. Good multi-disciplinary skills. Good record keeping skills Ability to use tact, diplomacy, persuasion in implementing change. Ability to deal with those with life limiting conditions. Effectively prioritises and coordinates own work. Skilled in teaching trained and untrained staff, carers and patients. Able to identify problems and recommend solutions Able to review current practice. Motivated to work collaboratively and enthusiastically as a member of a multi-disciplinary team Ability to remain calm and work effectively in pressure situations Able to be courteous at all times and to respect individual's dignity, integrity, cultural and religious beliefs Standard keyboard skills and ability to communicate through IT using packages such as Word, Outlook and Excel.
